Database for online entry of chart review of up to 2 years experience prescribing Suboxone®. Sponsored by an educaional grant to BioPsych Consulting from Reckitt Benckiser Pharaceuticals, Inc. The database included extensive validation of all data entry to ensure complete and accurate information. Navigation and editing routines were scripted to make data entry as painless as possible. Summary fields for data output were added making manuscript preparation simple and efficient.

Vine Street Center Intake

Database for online entry of complete clinical intake data for a licensed Opioid Treatment Program. Includes Addiction Severity Index, Drug USe Questionnaire, DSM-IV Substance-related Disorders diagnostic tool, Nicotine Dependence test, Michigan Alcoholism Screening Test, and several other psychiatric assessments. Includes automatic, real-time scoring of assessments. Developed with Filemaker Pro.
